Photographic ID: A valid passport.
Right to Work: A UK or Irish passport. If you were born outside the UK or Ireland, you must provide a share code along with proof of your right to work status.
A written reference dated within the last 3 years from a suitable professional referee,
The reference must include:
1) the referee’s contact details
2) confirmation of their professional relationship to you
3) the dates and duration of your employment or supervision
4) details of your responsibilities
5) and an assessment of your professional capabilities, conduct, and ability to work safely and effectively within an optical practice environment.
