If you have a police check already it must:
Display your job title as a locum pharmacist
Must have been issued in the last 12 months
Depending on where you are based there are different bodies who carry out the police checks and there are often two types of checks you can get, we only accept copies of the Enhanced or Continuous Update services listed below :
