This checklist outlines the mandatory documents and professional accreditations required for pharmacists.


Essential Documents

  • Photographic ID: A valid passport or driving license.

  • Right to Work: A UK or Irish passport, or a share code along with proof of your Share Code Status, which shows your eligibility to work in the UK.

  • Indemnity Insurance: An insurance certificate or a statement of facts.

  • Police Check:

    • England & Wales: An Enhanced DBS check and registration with the DBS update service.

    • Scotland: A PVG Scheme record dated within the last 8 years.

    • Northern Ireland: An Access NI check dated within the last 3 years.

  • Registration Number:

    • England, Scotland & Wales: GPhC (General Pharmaceutical Council) registration number.

    • Northern Ireland: PSNI (Pharmaceutical Society of Northern Ireland) registration number.


Accreditations and Training

Pharmacists need to upload proof of their accreditations and training to their profile. These include:

  • CV

  • Dementia Friends

  • EHC (Emergency Hormonal Contraception)

  • Level 2 Safeguarding

  • LASA (Look-Alike, Sound-Alike)

  • MUR (Medicines Use Review)

  • NADEX

  • NESA (National Enhanced Services Agreement)

  • NMS (New Medicine Service)

  • Repeat Dispensing

  • Risk Management

  • Sepsis

  • Smart Card

  • Smoking Cessation

  • Summary Care Records

  • Basic Life Support

  • Substance Use and Misuse

  • CPCS (Community Pharmacist Consultation Service)
